Rowan's Law - Concussion Protocol Acknowledgement

Rowan's Law - ACC Concussion Code of Conduct (2025-26)

ALL individuals who are 26 years of age or younger that are entering the facility (Ayr Curling) and/or participating in club sanctioned activities must comply with the attached PDF declaration.

Membership Selection Instructions

Registration Accuracy Confirmation

I hereby confirm that the information I have entered on this form is correct and true. I understand that if I have intentionally entered false information in this form to receive discounts that do not apply to me there will be a $50 administration charge on top of the outstanding membership dues owed.

PAYMENT NOTES:

All forms/fees are due by September 19, 2025 (for CYO) to enable league scheduling.
Any cheque or electronic payments returned NSF is subject to a $50.00 administration charge.
A late fee of $50.00 will apply to all full-season fees paid after October 3, 2025.
Preferred payment method is Debit or Credit (either online payment or in person); cheque or cash are also accepted.
